Job Description The Heart Transplant Coordinator oversees the multidisciplinary team in all aspects of pre and post transplant care in both inpatient and outpatient clinical settings and can assume clinical duties. The Heart Transplant Coordinator works directly with the Medical Director, the lead Heart Failure attendings, the Director of external transplant program partnerships, and the Director of Cardiogenic Shock and the lead Heart Failure NP to oversee and drive processes to improve patient outcomes.
