The Heart Failure Nurse Navigator (HFNN) coordinates the care of identified patients with Heart Failure in conjunction with the patient, primary care team and cardiology team, according to agreed-upon clinical guidelines, with the objective of coordinating high quality patient care throughout the hospital stay. The HFNN collaborates with case manager and social worker on identifying expected post-acute care needs.
