Job DescriptionThe University of Maryland Medical Center (UMMC) is a world-renowned tertiary and quaternary Advanced Heart Failure Center. The Heart Failure Clinic in the Coordinated Care Center at UMMC is a transition of care outpatient service that cares for the heart failure population. Emphasis is placed on safe transitions of care post-discharge from hospital admission for heart failure and the titration of guideline-directed medical therapy for those with heart failure. We seek an Advanced Practice Provider who will enjoy caring for this fragile population. Ideally, the candidate will have one year of cardiology experience, including an understanding of EKG interpretation and hemodynamic monitoring.
