DescriptionPOSITION SUMMARY:  Coordinates the Pulmonary Rehabilitation Program and provides proactive patient care management for all pulmonary patients and cardiac or PREP patients with pulmonary issues or where indicated or assigned.  Provides assistance to patients through evaluation and assessment, communication with third-party payors, care plan initiation, exercise prescription, and education.  Coordinates the smoking cessation program and support group.  Generates outcome data and reports for the Pulmonary Program including smoking cessation.RequirementsMINIMUM QUALIFICATIONS:  EDUCATION:  Successful completion of a CoARC accredited Respiratory Care Program required.
