The Licensed Vocational Nurse/Licensed Practical Nurse is responsible for direct patient care activities under the supervision of the Registered Nurse (RN). Completes assigned duties and responsibilities which involve performing nonstandard and specialized practical nursing activities, requiring broad work experience and demonstrated skill sufficient to resolve a range of nursing problems. Performs and documents focused/functional assessment and reports abnormal findings to the RN Team Leader, Charge Nurse, and/or Nurse Manager. As the Charge Nurse, the LVN/LPN, within their scope of practice, effectively assigns, manages and directs other LVN/LPNs and Nurses' Aides in the delivery of patient/resident care. The LVN/LPN demonstrates the ability to assign patients/residents care needs and the capability of the staff to meet those needs. Demonstrates knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report, and record relevant patient information. The LVN/LPN is the technical proficient in performing, reporting, and recording assigned nursing care, including, but not limited to: Administers oxygen (O2) therapy. Medication administration to include IV medication. Completes clinical reminders with appropriate consults and alerts as indicated. Ensures medications/supplies are not expired and removed from inventory. Accurately performs delegated procedures and treatments. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Electrocardiograms (EKG/ECG), lab draws; Data collection and documentation intervention of patient visit in applicable nursing note. Based on the patient's goals, reinforces patient and family health education, to include self-management, prevention and wellness. Assists in medication reconciliation. Reports all significant deviations to the RN and/or health care provider immediately. Applies skin tests and measures and documents results. Visual acuity tests. Closes encounters daily. Complete daily check of the automated external defibrillator (AED). Complete the Blood Glucose meter check daily, as applicable. Other duties as assigned. Adheres to standards of practice. Work Schedule: Rotating Shifts, Nights & Weekends, Compressed Work Schedule. 7:30 AM - 8:00 PM. 7:30 PM - 8:00 AM. Financial Disclosure Report: Not required Physical Requirements: Moderate lifting & carrying (15-44 pounds); reaching above shoulder; use of fingers; both hands required; walking (2 hours); standing (2 hours); kneeling (1 hour); repeated bending (1 hour); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and to 20/40 in the other; both eyes required; ability to distinguish basic colors; hearing (aid permitted). Preferred Experience: Successful Nursing practice with adult populations. One-year in adult mental health or substance abuse. One-year medication administration. Experience in patient education, adult education, or staff education. Basic computer literacy proficiency with the use of Microsoft Office Programs and the ability to learn new programs specific to the VA.
