We are seeking a Clinical Practice Coordinator of the Structural Heart Program. The primary function of this role is to work with multidisciplinary teams to provide clinical nursing expertise through all phases of care for patients with structural heart disease requiring interventional therapies.

Principal Duties and Responsibilities:
Provides nursing care and education to candidates for recipients of procedures within the Structural Heart Program.
Acts as a liaison between patients, families, hospital staff, attending physicans and referring providers.
Identifies candidates upon referral and interacts with appropriate staff to initiate and facilitate the evaluation process.
Collaborates with financial coordinator to ensure appropriate insurance authorizations are obtained prior to procedure.
Assures performance of necessary tests and studies based on the procedure to be performed to determine patients' candidacy and appropriateness.
Facilitates the transfer of evaluation information to multidisciplinary team members.
Participates in the multidisciplinary conferences to determine patient eligibility of procedure.
Serves as a resource for inpatient nursing and ancillary staff pre procedure and post-operatively.
Monitors and follows all pre and post diagnostic studies.
Creates, reviews and revises guidelines and protocols as necessary.
Participates in quality and performance improvement projects.
Collects data required for regulatory review and facilitates submission to the appropriate agency. Communicates report results to multidisciplinary team to include identifying opportunities for improvement.
Ensures NCD compliance where applicable and tracking of outcome and quality standards.
Maintains and updates list of potential candidates an prepares for weekly clinic meetings.
Coordinates system wide education events regarding preparedness for clinic procedures patients, including emergency services personnel, skilled nursing facility staff, visiting nurse staff, physical therapy and cardiac rehabilitation staff.
Manages outpatient care coordination to include outpatient appointments, attending the outpatient clinic visits, interrogation of devices as appropriate, performing dressing changes, and completing clinical documentation.
Participates in educational events to promote the Structural Heart Program including Grand Rounds presentations, educational summits, and marketing events as needed.
Serves as community liaison for education regarding structural hear program offerings and services.
Maintains inventory records of required equipment needed for procedures. Coordinates with appropriate clinical services team to ensure adequate supplies are available at all times.
Manages discharge workflow for post procedural patients, including discharge appointment planning and coordinating outside referrals.
Tracks and analyzes cumulative programmatic data and reports updates to Structural Heart leadership team.
Provides off hours coverage to assist with patient phone calls, questions and troubleshooting.
BSN or BSN in related field. Masters preferred.
Minimum of five years of cardiovascular nursing experience.
Procedural training and experience preferred.
BLS required. ACLS preferred.
Project management experience preferred.
